Release of Records
If you are a new patient and would like your records transferred to us, a release of records form will need to be completed. Please be aware, in some cases, medical records may be needed in order to provide continuing care. Also, current patients who are needing copies of their records will need to complete this form as well. For patient convenience, you can download a copy of the form below:
Scheduling
To schedule, cancel or reschedule an apppointment, you can call (605) 996-7526 Monday - Friday 8:30 AM - 5:30 PM or Saturdays 8:30 AM - 11:30 AM. We ask patients to cancel or reschedule their appointment 24 hours in advance if able. For procedures, 72 hour notice of cancellation or reschedule is appreciated.

We submit claims to all insurances and accept assignment for Medicare. However, insurance payments from Tricare will go directly to the patient. It is the patient's responsibility to bring those payments to the clinic. We are not an in-network provider for Avera Direct or Sanford True plans. We require a $50 down payment for new patients at their first visit which goes towards the balance not covered by insurance. For further explanation, please click here. Established patients with insurance will be expected to pay the copay at time of service. Established patients with insurance that do not have a copay are requested to pay $30 to go towards the services from the day of the visit. Any patients without insurance are requested to contact our business office to set up a payment plan.
